EIS Transition Entry for TCM and BHH Coordinators Begins Today!
Good Morning TCM and BHH Coordinators!
Today, you will be able to enter transition information in the EIS database for youth on your caseloads ages 16-20 with Intellectual Disability/Developmental Disability/Autism. The information we are asking to be entered in EIS is: Evaluation Status, Guardianship vs Supportive Decision Making, and referral information for OADS/OBH. We would like information to be entered and reviewed on a quarterly basis.
Over the past two weeks, several TCMs/BHH Coordinators have obtained access to EIS, but if you still require access, please have Supervisors with EIS access request for their staff via the electronic form in EIS, click here for instruction.
For any provider who does not have a Supervisor with access to EIS, please request access here: http://www.maine.gov/dhhs/ocfs/cbhs/provider/eis-staff-update.shtml.
