Waiver Provider Training on the
Service Implementation Plan

Sections 21 and 29

Please join the Department of Health and Human Services’ Office of Aging and Disability Services (OADS) on Wednesday, September 28th for a technical overview of the Service Implementation Plan (SIP), a new form attached to the DS PCP in EIS client data system. The updated Person-Centered Planning process was introduced to help individuals develop a vision for a “good life” and discover what it takes to live the lives they want to live. The SIP documents the provider’s service delivery strategies to support the person’s goals, includes timelines and strategies for implementation, and ensures clear expectations and adherence to the Person-Centered Plan (PCP) by providers.

This session will cover:
  • Reviewing the PCP for relevant service information
  • Proposing services based on goals
  • The SIP form and how to complete each part

This meeting will be recorded and posted after the event for those unable to attend.
